tv: failure state leaves camera on

The Port Noarlunga Reef stream failed but the camera was left on. This is not good and I think it could have been avoided. I'm not sure what happened, these are the logs of it starting:

What's weird is that the device is up check stops after 9:02, two minutes after starting.. I thought this is usually checked more often, especially during restarts.

This needs more investigation.

Start by tracing the broadcast and hardware state-machine transitions shown in the logs, especially the failure path after hardware reaches hardwareOn. Reproduce or test a failed stream startup and verify that the failure path leaves the camera hardware off.
