Add an example for clinical pipeline and stave

We need an example in ForteHeath to show how we incorporate clinical pipeline and stave.

Start by reading the referenced Forte pull request #793 and then inspect the ForteHealth repository structure for the existing example conventions. The work is complete when ForteHealth contains a runnable example showing how a clinical pipeline incorporates Stave.
