Title page information doesn't display correctly

### Description

The title and author information don't display correctly. A link to the appendix also seems to have been inserted into the title and the title itself has been repeated (with an extra -A added). The year was also inserted into the title when it shouldn't have been. The alignment and placement of the author names and corresponding author information are off.
