Author affiliations and contributions not visible

### Description

Superscript asterisks (*) and crosses (†) show up by the authors' names, but the matching footnote (indicating contributions) does not appear in the html page. The footnote does show on the first page of the PDF.

In addition (not sure if this is a separate problem or not) the PDFs show superscript numbers on the authors' names which refer to institutional affiliations in the footnote. Neither these numbers nor the footnote appear in the document.
