Dual-affiliations: one gets dropped

### Description

Both authors have dual-affiliations (using `authblk` package). 1st & 2nd for Author-I; 1st & 3rd for Author-II. But the common 1st affiliation disappeared in the HTML version. Author-I has only 2nd affiliation and Author-II has only 3rd affiliation visible.
