Author affiliations appear before title

The author's affiliations appear at the top of the articlem before the title. An example command in the TeX file was "\author[2]{Michael S. Kelley}\affiliation[2]{University of Minnesota, Minneapolis, MN, USA}".This resulted in "2][University of Minnesota, Minneapolis, MN, USA" appearing before the title.
