Rename proxy.config.http.default_buffer_size to default_buffer_index

In order to be more consistent with our other index-based buffer size variables, the `proxy.config.http.default_buffer_size` parameter should be renamed to `proxy.config.http.default_buffer_index` or similar; the trailing `_index` being the most important detail.
