Default skill creation flow should produce .skill package, not just SKILL.md

Claude frequently produces only a SKILL.md file when asked to create a skill, even after reading the skill-creator skill. Users who don't know to explicitly request the .skill package end up with incomplete skills that don't work. The default behavior should be to run the packaging script and output the complete .skill file.
