Description
**Bug Description**
The eager beaver mode is getting on my nerves. When I give precise instructions, I want it either done that way or at least a question if I want Claude to do some kind of stretch goal. What's even more infuriating is that when I told Claude to roll back the two things it added without asking, it went to document that in the README.md which would end up reading like insanity.
TL;DR - don't add anything that I didn't ask for without checking and don't document the removal of the extraneous things that I didn't ask for in the first place!

Research direction
Start by tracing the Claude Code behavior that makes unrequested additions and then documents their rollback in README.md; the issue does not name an implementation file or test. Done means the tool asks before stretch work and does not add removal notes for changes the user did not request.
