[Bug] Claude Code ignoring user instructions and modifying approved requests

**Bug Description**
Claude-code has repeatedly violated every request i have given. Despite each round being checked and confirmed. Afterwhich it changed the request, either writing new request not approved or adding content not requested. That's a ton of wasted tokens and money. I am completely frustrated because I am unable to get claude to comply to anything. AAARRRGGGG!!!!!!
