[Bug] Agent fails to report output file location and writes to inaccessible directories

**Bug Description**
The agent routinely completes work and writes output to a markedown file like a runbook i need to follow by hand - and fails to do the obvious thing, even tell me the name of the file or where it put it. or it puts it in /tmp directory under scratch pads etc. instead of someplace user accessible.
