Description
On every new PR commit, >3+ comments get written on the PR. Can we improve the [example](https://github.com/anthropics/claude-code-action/blob/2e6fc44bd4c8c7f287e83a4dccc480a220f8ae4a/examples/pr-review-comprehensive.yml) such that it updates reviews and comments in place?
A PR with 10 commits ends up with 30+ comments
