Custom MCP server with Salesforce is falling to Authenticat

### Describe the issue

When a user click connect he is being passed to SFDC login screen, login successffy and being transferd back to claude and get this message.

We have same people with same permmissions that it is already working for them and the issue surfaced with new users.
I already verified all the mcp server external client app in SFDC sets correctly.
I dont even see the login attempt of the user in SFDC when they get the error on claude
So it seems like a communication issue between Claude and the Salesforce server
