Error from provider (Console): Upstream request failed: [invalid_request_error] This request requires an end-user identifier. Provide a non-empty safety_identifier (or user) field.

Steps to reproduce
- Open Windows PowerShell.
- Run OpenCode version 1.18.19.
- Select or use the model provider/build
Muse Spark 1.2 Free. - Type a simple prompt (e.g., "hai") and press Enter to send the message.
- The terminal immediately throws the upstream provider error repeatedly.
