[FEATURE][DESKTOP]: Display more information for failed (permission asked / original value before failed, for shell, edit, tool, mcp, webfetch, etc)

Hello, it seems errors on opencode desktop need improvements.
Often we don't see what the previous shell, tool, mcp, webfetch was. It is true when permission is denied, or more generally when tool failed.

Some non exhaustive examples :

This issue could be great to gather all failed messages lacking information.

Improvments

Could be great to add more informations, like :

• original path for files,
• status code webfetch,
• permission asked or permission that make it failed
• previous shell command
