AMP Registration Form Fill up email field

I am trying to enable amp support to our client, as part of the activity we have to fill the registration form to register the sender email address. As part of registration form, we need to provide the details for email, and name. Could you please help us to know whose email and name should be provided (client or our personal email address)? Also help us to know the purpose of providing the email address.
