Include timestamp in application name when running migrations on postgres

When running migrations, it would be useful to include the timestamp in the application name when running migrations on postgres, as the application_name can be logged out and/or used in auditing.

Example: `dbmate[20251111103123]` for a migration script `20251111103123_update_data.sql`
