数据源面板处编辑面板处请求头和请求参数的表单项删除异常

数据源面板处编辑面板处请求头和请求参数的表单项有多项时，删除中间项，其对应value没删除，只删除了最后一项的value

Steps to reproduce the behavior: / 详细复现步骤：

1. 打开 [demo](http://lowcode-engine.cn/demo)；
2. 打开数据源面板；
3. 点击新增或编辑已有数据源；
4. 在存在多条请求头或请求参数时删除非最后一条
5. 只会删除最后一条的value

删除项请求参数与请求头信息的表单项时，对应项的name和value应该一同删除
