First line missed in concatenated md files

Execution of following statement results in single pdf file.

```
const documentation = [ ... ]
const bookPath = '';

markdownpdf()
.concat
.from(documentation)
.to(bookPath, function () { console.log('Created', bookPath) })
```

The problem here, that the first line of all concatenated files is missed in final pdf.
