Adding a Pet Name Section in Filling form

If we Select a Pet , and at the Stage of Filling the form there should be a Section for the Pet Name as Same as the Pet Name which was on the homepage ,by which we are selecting the page.


**Describe the solution you'd like**
After Selecting any pet, in the filling form there should be a photo or name/info related to the selected pet, People will be able to see that what they are paying off.

**Describe alternatives you've considered**
We Can also do one thing that the ,if we select a pet ,then in the page the filling there should be any identity of the pet
