Acceptance Criteria:
- Identify root cause from logs
- Reproduce locally and add tests
- Fix regression(s) and flaky tests if any
- Ensure CI is green on PRs and main
Deliverables:
- PR(s) with fixes
- Notes on root cause in issue comments
