Description

To quote :help gv:

In Visual mode the current and the previous Visual
area are exchanged.

Did you know that? I didn't know that. Kinda nifty, actually.

Research direction

The issue does not name files or tests. Start by locating the Visual mode key-handling entry point and existing key mappings, then compare the requested behavior with Vim's :help gv documentation. Done means pressing gv in Visual mode exchanges the current and previous Visual areas, with coverage for that behavior.
