Description

Currently, it puts the cursor at the first column.

This is also an issue with <<, and >>.

H, M, and L seem to ignore startofline and put the cursor on the first non-whitespace character regardless.

Research direction

Reproduce the cursor behavior for dd, <<, and >> with startofline disabled, then compare it with H, M, and L. Trace the keybinding or command entry points responsible for these motions and verify that the completed behavior preserves the cursor's horizontal position where the setting applies.
