`<leader>, t` mapping doesn't always get applied

I'm finding a mapping of <leader>,t is not always being applied.
This is my config:

And hitting <leader> t sometimes executes the command (I've tried many commands and verified it's not the command.
It seems to work until I edit a file, at which case triggering the mapping does nothing. I've tried changing leader from its default, I've checked for any conflicting keymaps and no luck in figuring out what's going on.
I'm on VSCode 1.3, extension 0.17 and on High Sierra.
