Research direction

Start by reproducing the issue with Shift+Arrow and Shift+mouse-left in the reported VSCodeVim and VSCode versions, then trace the extension's keyboard-selection handling. Done means Shift+Arrow selection enters visual mode consistently with mouse selection, with a regression test covering the behavior.
