Add report form or other simple contact possibility

### Proposal

Create a form like a google form wit questions

It should be as easy and simple as possible to have a low barrier

- google forms? :(
- form on zottelig.ch (spezific for osm apps)? :|
- (with donation button)
- own side? contact.osm-apps.org?
