VCF/Sample create patient fixes/improvements

* There is no plus on sample page
* The create button should be next to the cancel button (as it scrolls off the page otherwise)
* If you add a patient, it doesn't actually link them until you click save. Maybe we should just save it against sample straight away (or at very least show a need to save message)
