Research direction

Locate IvState, SetIvState, and iv_state_test! in cipher and crypto-common, then compare the existing SetIvState tests in the block mode crates. Done means the traits are moved into cipher, coverage is added to iv_state_test!, separate block-mode tests are removed, and SetIvState is merged into IvState.
