Description
Issue Description
When A TCR is created it stores all the data requires to create the task, later updating Github issue does reflect the changes on TCR page but still creates the task with the initial data before TCR getting approved.

Research direction
Start by tracing the TCR approval and task-creation flow in the repository, focusing on where GitHub issue data is read before approval. Reproduce the sequence of updating an issue title, viewing the TCR, and approving it. Done means the created task uses the latest issue title rather than the title captured at TCR creation.
