Description

When a tool is set with approve in the mcp server it just rejects the tool call. It should prompt the user for permission in the ui.

Research direction

Start by reproducing a Supabase MCP tool call configured with approval and trace whether it is rejected before the UI can show a permission prompt. Compare this behavior with other MCP servers or tools, then verify that approved calls prompt in the UI and proceed after permission is granted.
