Create a formula-free disabled flag for user entry in the app

As it stands, the only trigger condition for SSI is blindness - perhaps this should be replaced, at least at the UI level, with a broader SSI-eligible flag? This way we would not have to ask for specific info and also fix confusion around what should trigger it.
