Research direction

Start by locating the cookbook documentation and reviewing the no_default_middlewares change in #1459 to understand how header manipulation works in the Plack stack. Add cookbook examples covering ETags, ContentMD5, and similar header-related uses, with the documentation showing how each is configured.
