Maybe there are some more but these are the countries that i have seen that are the same but appear twice:

11th - België - Belgique - Belgien

Belgium

4th - The Netherlands
-
28th - Netherlands
Within a country, the same province may also exist 2 times:
4 th - The Netherlands > 2nd - South Holland
4 th - The Netherlands > 4nd - Zuid-Holland
These countries & provinces should be merged.
