Research direction

Reproduce the failure with Nginx Proxy Manager v2.11.3 using the jc21/nginx-proxy-manager:latest Docker image and an expired Let's Encrypt certificate. Start with the manual renewal action and its "Internal Error" response, then compare it with deleting and recreating the certificate. Done means renewal succeeds without deleting the existing certificate.
