Manage the permissions related to my connections

As a dapp user, I'd like to manage the permissions related to an account, so that I can revoke a site's ability to do something that I don't trust it with.

**Description**

This story is a context menu for managing the permissions of all sites connected to a specific account. This is an MVP iteration. In a future story, we'll be implementing the ability to manage the permissions on a per site basis.
