Description

If possible, it would be great to change two parts of the second page of the question form. Right now the second page of the question form looks like this:

![MicrosoftTeams-image](https://user-images.githubusercontent.com/88652868/129173171-c6c7cd8c-8bef-45a4-a168-1375b6bf593d.png)

Firstly; in stead of 'User email' we would like to have it in Dutch: "Jouw e-mailadres".
Secondly, we would like to have additional form lines asking the questioner for their name, age and location to publish alongside the eventual article. Included in this change should be to change the header text to reflect them giving their consent to publish their question. It would change the complete sentence to:
"Laat je e-mailadres achter als je op de hoogte gehouden wilt worden. We noemen ook graag jouw voornaam, leeftijd en woonplaats onderin het artikel."

We would prefer if the text boxes have a suggestion text "Optioneel" to reflect the optional nature of the request.

It would look like this:

Might be possible to combine this with #95

Research direction

Start with the second page of the question form described in the issue and compare its current fields and wording with the requested Dutch text. Identify the form entry point and any existing coverage before changing the email label, consent text, optional fields, and publication details; done means the rendered form matches the requested layout and behavior.
