Move Normative Text Out of Appendices

We should:

1. Clearly document which appendices are normative and which are informative. Are all appendices informative?
2. Move the normative text from Appendix C out of the appendix and into the main spec.

Start with the discussion linked from issue 1467, especially the referenced comment, and review the appendices to determine which are normative or informative. Identify the normative material in Appendix C and the appropriate locations in the main specification; done means the appendix status is documented and that material no longer remains in Appendix C.
