Description
I'm planning to make a pull request with updates for the OMOP CDM v5.4. To preserve support for 5.3, I might just have it check the julia environment for a value of CDM_VERSION, keeping 5.3 as the default at least until the next major update.

Research direction
Review the existing Julia implementation of the OMOP Common Data Model and determine where the current v5.3 behavior is defined. Then examine how a CDM_VERSION value could select v5.4 while preserving v5.3 as the default. Done means the implementation supports the v5.4 updates without removing v5.3 support.
