Description
To reproduce run the following command:
ipy -S -c "raise Exception" > test.txt 2>&1
The line endings are all \r\r\n. This occurs because the generated strings use \r\n but it gets piped through the TextIOWrapper of which translates \n to \r\n.

Research direction
Start by running the reported ipy -S -c "raise Exception" > test.txt 2>&1 command and inspect the console-output path involving TextIOWrapper. Trace where generated strings receive \r\n; done means redirected output uses single, correct line endings rather than \r\r\n.
