Fix form layouts on Budget Proposals

1. Fix the form layouts so that:
Questions are clear and well written and use consistent terminology and voice.
Options are easy to select (radio buttons instead of combo boxes)
Hierarchy of information is correct (currently type sizes vary randomly)
Instructions are not in the boxes, but above them.
The errors are clear and show the users what they need to correct
