Request for a Questionnaire/Form

Me and my team are working on annotating scientific research publications by inviting different annotators that give their inputs to the ML Model suggested labels for individual research papers after reading the abstracts.

I want all the annotators' information for me to choose which annotators to consider and whom to not. It would be great to have a questionnaire/form that can be filled by the annotators that can be read by me and my team to help us choose the best annotators after getting to know their background, field of expertise etc.

An alternative we have considered is to have a link to a google form upon sign-up. Please let me know if such a capability already exists within Label Studio or if it's present in the pipeline for further development.
