how is "memory lost"

in reality I suspect nerves and synapses don't come and go too much, yet we loose information. We do need to understand how information is retrieved, but he question here is if synapses perhaps atrophy in certain conditions, and eventually could leave a nerve unconnected and up for reuse.

So the core question for this issue is if synapses disconnect and under what circumstances.
