Content Pack Duplicates in List

Seems like there may be two similar issues on the content packs list:
- Each version of the content pack is listed as a separate content pack
- Paging back and forth on the list creates duplicates
## Expected Behavior

Each content pack is listed only once with its version only visible if the content pack is selected.

Each version of the content pack is listed as a separate content pack and paging on the list view creates duplicates
## Possible Solution

## Steps to Reproduce (for bugs)

1. Go to System > Content Packs
2. Create a new content pack with any entities (More Actions > Create New Version)
3. Once you have multiple versions, go back to System > Content Packs and see that both versions are listed as individual content packs
4. Page back and forth to the page your new content pack is on
5. Duplicates start to appear at the top of the page
