Add three text boxes for yes/no/maybe why text

The user should be able to have three different text boxes for the why text - one for yes, one for no, one for maybe. These should appear when the user checks the corresponding checkbox. This should also show the answers separately in the output file.

Having the user put all their answers in one text box is confusing and does not mirror the setup of the GM forms.
