Proof of concept - create an ingest form for software review requests that could create tickets using the SN API

This is in reference to the CTO wanting to transform the SRR process. We might be able to use GEAR as the ingest point for users to submit software review requests in order to utilize the tech_catalog library, automate the populating of GEAR with the new IT standard object as "proposed", and then using the SN API, automatically create a SRR ticket.

We'll have to talk to the SN team about their API to create tickets.
