Description
Issue Name: IdentityThefteta.gov_11_2015 - Issue - IdentityThefteta.gov_11_2015 - Issue - Text on affidavit is confusing
Device Make: Apple
Device Model: iPhone 5S
Device Operating System Version: iOS 8.3
Browser: Safari
Carrier (or WiFi): ATT
Severity: Low
Description:
On the Identify Theft Victim's Complaint and Affidavit form the help text box on the right of the forms seems to be missing words and is possible indicating the wrong element.
Steps To Reproduce:
1) Complete the ''Report Identity Theft to the FTC' through to the victim affidavit.
2) Scroll to view the right side of the affidavit where a box of help text will display.
3) The beginning of the help text is confusing and I think maybe should be referring to the Social Security field.
Will attach screenshots.
Expected result: Display meaningful help text.

Research direction
Reproduce the Report Identity Theft to the FTC flow through the victim affidavit on the reported iPhone/Safari setup, then inspect the help text box on the right side. Compare its opening text and field reference with the displayed form; done means the affidavit shows meaningful help text referring to the correct field.
