Description

Issue Name: IdentityTheft.gov_11_2015 - Scenario 2 - Submission Error

Device Make: Apple

Device Model: iPhone 6

Device Operating System Version: iOS 9.1

Browser: Chrome

Carrier (or WiFi): AT&T

Severity: Medium

Description:

While completing scenario 2, I received an error "submission error".

Steps To Reproduce:
1) Go to test home page.
2) Click get started.
3) Click I want to report identity theft.
4) Check the box beside "credit card accounts", click continue.
5) Check the box beside "open a new credit card account", click continue
6) Click continue under "this will help you fix problems..."
7) Enter theft details, click continue
8) Enter your information, click continue (actual steps get fuzzy here)
9) enter suspect information, click continue (actual steps get fuzzy here)
(not sure if additional steps occurred)
10) Received popup "submission error".

Note: I tried this with two separate emails, to be sure it wasn't one that I had used previously. I also did this two days after my previous attempt, so my understanding is that no previous accounts should have existed for me in the test environment.
