New HR Links Separation Codes: EXP, MNP

Add the following codes to the lookup_separation table:
Fields: (id, separation_code, separation_description, date_created)
Values 1: (134, 'EXP', 'Exp of Appt', current_timestamp)
Values 2: (134, 'MNP', 'Moving to New Provider', current_timestamp)
