Allow create issue without comment

Would be nice if plugin has support for creating issue without the comment (from the window toolbar/contextmenu/whatsoever).

It can works in two ways:
1. Insert a comment to a caret position after issue was created
2. Just create issue and do nothing (this was useful to create an issue in IDE without needs to go to browser, and create it manually)

Start at the existing issue-creation flow exposed from the IDE window toolbar or context menu, and trace how it currently inserts comments. Clarify whether the desired behavior is inserting at the caret or creating the issue without changing the file; done means the selected behavior creates an issue without requiring a browser.
