When FTA slack integration is enabled make "send to slack" default

**Is your feature request related to a problem? Please describe.**
When the Slack integration is enabled on the FTA notepad, make the default behavior send to slack when creating an issue, and let me uncheck it if I don't want to send. Out of all the issues we logged on Edison there were only a small handful that weren't sent to slack, and of those at least half probably should have been.

**Describe the solution you'd like**
With the workflow of FTA Notepad + Slack, there is rarely if ever a time to not send the message to slack. The only time I can think of is when it is an issue that is resolved immediately, doesn't need follow up, but does need to be logged (e.g. wrong channel which is changed when discovered, but logged for tracking purposes).

Always remembering to check the "Send to Slack" option when logging an issue.
